Where it asks for the students address history, they need to list all addresses since age 13The social security number is required by the state.  Without a social security number, the paperwork cannot be processed.  This forms must be fully filled out.  Every blank needs to be filled in.  Only use nicknames where asked to.

Preliminary Information:

The following papers are required before leaving the school on the field placements. 

Unit 1: Teen Parenting-  Objective: To examine the short and long term effects of being a teen parent.

Unit 2 Pregnancy: Objective -be able to identify the stages of pregnancy and the developing baby during prenatal development.

Optional Assignments – See me if necessary

Unit 3: Infants – Objective: Identify how the infant child’s physical, emotional and intellectual needs change and develop and over the course of the year and be able to meet these needs as a parent.

Unit 4: Toddlers-Objective: Identify how the toddlers physical, emotional and intellectual development progresses and changes over the next two years
