- Parent Letter
- Baby Think It Over permission sheet (The baby is very expensive. Therefore, you need to sign off before your child can take the baby home over night home) Baby Think it Over Program PPT, The “Baby Think it Over Program” worksheet
- Empathy Belly Parent Permission (The belly is very heavy and we want to make sure you know about the experience before your child participates.)
- Each student MUST complete a background and felony check.
(Example 1 and Example 2 ) These sheets are required for the students to volunteer at the day care centers. Where it asks for the students address history, they need to list all addresses since age 13. The social security number is required by the state. Without a social security number, the paperwork cannot be processed. This forms must be fully filled out. Every blank needs to be filled in.
