Technology Violations:

All students sign a Responsible User Agreement each time they log on to their device. Their sign in acknowledges an understanding of the district expectations.

Use this form when a student has violated one of the items in the agreement. The teacher may complete the form with the student or you may refer the student to Administration or Sara Meier for completion. (Depending on the offense)

Teachers may fill out SECTION 1 of the following form with a request for student to conference with either Dr. Czapla, Mrs. Ahlers or Mrs. Meier (MSTL) and submit only SECTION 1. If preferred, teacher may complete SECTION 2 of the form with the student.
